Under Investigators, select AD-416/417 Research Resume and Classification with CSREES - 2008 Assurance Statements

Select GA on the US Map

(The AD-417 and CSREES-662 forms are always completed and submitted along with the AD-416, which must be completed first.)

Select GEO – University of Georgia, Agricultural Exper Stations as your home site
Enter "tiger" as the password
[Proceed]

Enter the P.I. last name and [Display] the work list of projects for you as the principal investigator. Select one project on the worklist and select AD-416 Research Resume and [Proceed]

If you are initiating a new AD-416 form, select one of the following:

NOTE: the funding source determines which type of AD-416 you initiate.

  • New Formula Funded or State Project
  • New Grant/Coop Agreement
  • Revised AD-416 for an existing project not currently in the work list. If you have a copy of the original AD-416, find and enter the project’s CRIS accession number, or contact Liz McCaleb @ 706-542-9005 for assistance.

[Proceed] and enter the following information:

  1. Project Number: GEO###### - Please call Liz at 706-542-9005 to get a number (In Griffin or Tifton call the Asst. Dean's Office)
  2. Status:
    A = NEW, projects to be submitted for approval
    B = EXTENDED, if you have received an extension date
    C = REVISED, projects already documented in CRIS that are being modified
  3. Title: Enter the title for this project. Titles are limited to 100 characters, including spaces. Do not use phrases such as "research on" or "investigation of", etc. Do not use quotation marks.
  4. Performing Department: Select the home department of the Principal Investigator for the Performing Department
  5. First/Second Cooperating Departments: Select up to 2 departments participating significantly in the project.
  6. Multi-State Project Number: (if applicable) The regional project no. identifies projects supported by Regional Research funds, or those formally on record as contributing to a Regional Research project. This no. is in the format: region-### (i.e. NE-999)
  7. Cooperating States: Select any OTHER STATES that are participating in the same research project from the drop down list.
  8. Investigator Last Name: List the Principal Investigator’s last name and at least one first initial. Lines 2-6 are used for co-investigators.
  9. Project Contact: This may be the institution’s name, director, the P.I. or public relations contact.
  10. Project Type: This field is required in order to complete an AD-417

    If you are entering a new Grant/Coop Agreement this additional information is needed:

  1. Contract Number: A contract number is assigned by the granting agency. Enter the assigned contract number. The contract number can be found on the Agreement Face Sheet if one is available. If no contract number has been assigned, enter the words "NOT YET AWARDED".
  2. Proposal Number:  This number is assigned by the granting agency.  Enter the assigned proposal number found in the award letter and on the Agreement Face Sheet. 
  3. Amount: The amount is determined by the granting agency. Enter the amount designated by the granting agency, as shown on the Agreement Face Sheet. If the grant has not yet been awarded, leave this blank.
  4. Fiscal Year: Enter the Fiscal Year for this grant information using the 4 full digits of the Federal fiscal year October 1 – September 30. (i.e. 2004)
  5. Award Date: The award date is determined by the granting agency. Enter the award date as shown on the Agreement Face Sheet, in the form MM/DD/YY. If the Grant has not yet been awarded, leave this blank, the amount has not been determined either.
  6.  


  7. Start Date: Enter date research will begin as MM/DD/YYYY. YOU CANNOT ENTER A DATE THAT "STARTS" BEFORE PAPERWORK IS COMPLETED. (i.e. If we receive your proposal in the office on 09/01/2003, you cannot set the start date any time prior to that.)
  8. Termination Date: Enter the estimated termination date for the project as MM/DD/YYYY. (CRIS will automatically calculate the duration in months for you)
  9. Objectives: Enter a clear, concise statement of the objectives of the research, which should be specific and attainable within the duration of the project using the resources available. Limit to 3200 characters.**
  10. Approach: Describe the ways in which the research is to be conducted, with emphasis on the scientific methods and any unique aspects or significant departures from usually approaches. Limit to 3200 characters.**
  11. **We recommend that you paste in the text from your word processor (spell-check there), but do not include formatting such as tab, indent, bold, underline, super/subscript, nor any graphic symbols.

  12. Non-Technical summary: Include both the situation this research addresses and the purpose of this project. Limit to 400 characters (2 or 3 sentences) total. The 2 sections will be combined into one short paragraph when displayed. Use complete sentences.
    1. Enter the situation or problem the project addresses.
    2. Enter the purpose of the project.
  13. Keywords: Enter one per line. Limit to 20 lines. Do not use hyphens to link phrases. It is important to assign appropriate keywords for automated information retrieval of project information.

When all information is entered, [Check Data] and note any errors. View the displayed information carefully. You may make corrections or additional edits by using the [BACK] button on your browser toolbar. [Check Data] again.

When the information is correct and complete, [Save Information]. You should see a Confirmation Screen if your work was successfully saved in the work file.

Print a copy of the Confirmation Screen.

You must use the [Return] button provided to return to the Select Project screen from the Confirmation Screen. This will allow you to select another form to work on.

Do NOT use your browser’s back button! If you use your browser’s back button after saving your work, you may not see the project listed, or worse, if you press the [Save Information] button again, you will create a duplicate project entry.

Please be sure to use the [Return] button provided at the top of the Confirmation Screen.
