This form is submitted annually to report progress during the life of a project. The same form is used to submit a termination (or summary) report, which should be completed within 60 days of a project’s termination. The information required for the AD-421 is:

• A narrative summarizing significant results, accomplishments, conclusions and recommendations aimed at scientists, administrators and legislative officials;
• A brief 2-3 sentence non-technical summary of the project’s expected economic, environmental, and/or social impact;
• A list of significant publications issued during the reporting period.
